Please note that the forms provided are in Microsoft Word software, version 97. Areas on the form are
protected so that you should only be able to "fill in the blanks." Use the mouse or Tab key to move around
on the form (as shown in the figure below).
On these forms, you cannot continue from one blank line to the next without using the mouse or Tab key. So when
you complete a line for a longer answer, you need to move to the next blank as shown below. In other words, you
cannot move from Line 1 to Line 2 without using the mouse or Tab key.
When you have completed the form, you can print it. You may also want to save it to your computer for future
reference. To save it, click on File and Save As (as shown below) and then save it to a location
of your choosing.
If you do not save it, when you click the Back button, you should be given the opportunity to save it
again as shown below.
