Christian Family Services - Forms The following forms are required to complete your adoption package. Although these forms are not required with the Response Form, Adoption Application, and application fee, they are needed before we can show your portfolio to birthparents. We also have provided some additional forms below that have information in which you may be interested. Please note that we have provided these forms in two different formats. If you have Microsoft Word software on your computer, you can fill the forms out on-line, print them, and send them to us. (You might also want to electronically save a copy to your computer for your records or future use.) You can click here for a detailed example of using the Word version. If you do not have Microsoft Word software, printable Adobe Acrobat (.pdf) files are also available. These types of files are commonly used on the Internet and the Acrobat Reader software is available for free download at the Adobe Acrobat website located at http://www.adobe.com/products/acrobat/readstep2.html.
